Referral Method
This Community Links Service offers support in the community for people that are currently accessing Secondary Mental Health services and experiencing social isolation due to mental health distress. This service is offered to all people over the age of 16 years.
Current Activities
This service offers the opportunity for people that have become socially isolated to gain confidence and participate in activities and events of their choice within their local community, with the support of a Community Links Worker.
A Community Links Worker will meet with the client and discuss support requirements, goals and plans. Regular support sessions will be booked, usually on a weekly or fortnightly basis. Sessions may be offered for an hour or two hours dependant on the requirements of the client.
Sessions include gaining confidence with public transport, developing strategies for leaving the house, visiting crowded places, accessing training or college courses, taking part in leisure activities such as visiting the gym, going swimming and visiting cafes.
Future Plans
The team at Portsmouth Mind are interested to know what you like about the service and what you feel would improve it. We are constantly striving to improve the services we offer. For example; the Community Links service no longer has an upper age limit and we are looking at ways to make it easier to refer into the service.
Whilst some services are still free of charge, Portsmouth Mind welcomes referrals from people that wish to self fund their support, for example through a Direct Payment. Please ring us to discuss your options.
