Portsmouth Outreach Team
The Port team are a small team of ten staff who have been set up to meet the needs of a small but vulnerable group of service users who have a severe and enduring mental health problem and have had a history of difficulties engaging with the standard mental health services.

The Team currently operate Monday to Friday 9- 5pm , with access to the Crisis Resolution and Home Treatment Team out of hours.
Who we are
The Team Manager is Roger Batterbury, The Senior Practitioner is Jane Townsend and the Consultant for the Team is Dr Elizabeth Caesar.
The Team are made up of Registered Nurses, Social Workers, and Support Workers. They also have Student Nurses and Social Workers on placement from time to time. They also have close links with the Occupational Therapy Consultant Peri Hanzouli.
What do we do ?
The Team closely follow the National Service Framework model and Policy and Implementation Guide for Assertive Outreach Services and try and find creative and client led ways to engage with Service Users. They are very much a recovery focussed service.
They offer both practical and specialist input, in almost entirely community based settings. They can deliver medication, and administer it at home, they can support with domestic chores and shopping, finances, and decorating are possible examples. This is needs led and would be part of a recovery plan.
The Team are essentially community based but do support Service Users who have entered the residential services wherever possible.
Referrals
The Team mostly take referrals from the Community Mental Health Teams and the Residential Services.
